The Macmillan health Care Service has an established team with a long history and was expanded in 2013. National evidence suggests that patients would prefer to die in their own homes than in hospitals. This service provides a robust and consistent package of care for the patient and families seven days a week and 24 hours a day. Successful bids have enabled the service to include night and afternoon sits over 7-days to patients and families.

Around 1 in 3 deaths occur in patients over 80 years of age. The profile is changing with a large amount of deaths occurring following a chronic illness such as heart disease, diabetes, cancer or strokes. The Macmillan Health Care Service is a central point of referral for End of life services. The team works in collaboration with the District Nurses, therapist, General practitioners and Macmillan Nurses. The Health Cares are passionate about the service they provide and established a philosophy that underpins their service.

This is an exciting opportunity for an individual who wants to be part of a compassionate team who delivers care to End of life patients. The post holder is responsible for; supervising and coordinating the MHCT staff and ensuring patient care is carried out according to the care plan and as delegated by the registered nurse, assisting in the induction of new staff, conducting supervision of junior staff members, assessing competencies of staff within the team, completing rotas as required and managing referrals into and out of the service using specific criteria, carry out delegated work in accordance with the competencies detailed within the NQF at Level 5 foundation degree level, working as part of a specialty team, the post holder will offer support to ensure patients are afforded a professional and caring service within the necessary clinical timescales, patients to be tracked through the process to ensure they receive care at appropriate times, therefore there will be an element of validation and auditing required ensuring that patients are treated in a timely and efficient manner.

The nature of this post requires the post holder to have a high level of tact and diplomacy as well as strict adherence to the rules of confidentiality. This requires the provision of a robust service: the post holder may therefore be expected to work on a rota basis ensuring the service is appropriately covered at all times across a seven day week and will be required to work on their own initiative, in accordance with Trust policies and procedures, referring issues to the Patch Team Leader as appropriate.
