We are looking for an enthusiastic and self motivated youth worker who is passionate about helping young people (YP).

The successful applicant will work alongside the paediatric renal multi-disciplinary team to help support the emotional, psychological, and social well being of YP with chronic kidney disease (CKD).

This is a three year fixed post. It is expected that the dataset will provide the hard evidence needed for on-going provision of youth worker support so that the post can be permanently incorporated into NHS core funding.

A particular focus of the post will be the transition period from paediatric to adult care. Specialist knowledge of kidney disease is not necessary but candidates should be able to display knowledge of the challenges facing adolescents with a long term medical condition.

The post holder will:
• Play a leading role in the detailed planning of activities and interventions to support young people moving from paediatric to adult care
• Be confident to lead in the delivery of recreational, social and general support services for YP with CKD.
• Be able to feed back to the multi-disciplinary team where there is cause for concern about the well being of individuals
• Be able to work autonomously, organising and facilitating individual and group activities.
• Help with the development of training and educational resources for YP and healthcare professionals to improve their knowledge and awareness of the issues faced by YP living with CKD.
• Will provide opportunities and mechanisms for YP to offer feedback about the care they receive and the issues that they are facing, and where appropriate suggest and implement changes.
• Innovate approaches to communicate with YP about the work of the service e.g social networking
• Identify and share best practice for youth worker support for YP with CKD nationally and internationally.
• Be an integral member of the ‘Ready Steady Go’ transition steering group and will work collaboratively with other transitional teams within the Trust and other Youth workers across the UK
• Help develop a dataset to provide evidence of the impact of a youth worker to include patient experience, health and social outcomes.

The post is full time, 37.5 hours per week. Requests to work on a part time basis will be considered. Hours of work will be flexible to meet the needs of the clients.

Getting to Work

Application can be made for parking on site at UHS or through one of the park and ride schemes.  This will be based on specific criteria to determine eligibility for site parking as this can be very limited.  UHS encourages staff to utilise other green methods of getting to work such as cycling or walking where possible.   UHS is well connected through bus services to the city centre and other areas.
