About the Child and Youth Mental Health Service

The Child and Youth Mental Health Service (CYMHS) at Children's Health Queensland specialises in helping infants, children and young people up to the age of 18 years who have complex mental health needs.

CYMHS works collaboratively and in a recovery-oriented way with infants, children and young people and their families. This means we recognise and help build on individual and family strengths for better health and wellbeing. By working together, we support children, young people and families to get back on track to reach their optimal mental health.

CYMHS has a number of teams across the Greater Brisbane and Pine Rivers regions. We serve our local communities, and we outreach across Queensland through some of our specialist programs. We provide community and hospital based services, outreach and telepsychiatry programs, early intervention, forensic and other specialty services.

Each year CYMHS takes pride in providing quality mental health services to more than 2500 Queensland children and young people, and their families.

Your Opportunity:

The Nurse Navigator (NN) – Paediatric Transgender Health is a Registered Nurse accountable for the provision of specialised knowledge and advanced clinical practices supporting patients and families transitioning from paediatric gender services to primary care and adult services and leading the coordination of integrated care of gender clinic patients outside of the metro Brisbane area. Gender healthcare involves transdisciplinary care and this role will augment existing models of care at LCCH and provide a nurse-led model of care for a sub set of this population with complex care and coordination needs.  The NN role focuses on patient-centred health care and aims to facilitate the patient journey through the complex health system. This role aims to lead and facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and families and coordinating patient care. The NN role utilises advanced clinical and professional skills and knowledge to deliver a positive patient centred health experience for individuals. 

The NN – Paediatric Transgender Health, will also work collaboratively with other NN's in CHQ and the CHQ Connected Care Program. The NN is accountable to provide safe, quality and cost effective family-centred paediatric nursing care that is consistent with contemporary standards.
