Contract Type:  Fixed term for 2 years

Hours per week:  37.5 hours

An exciting opportunity has arisen for a pharmacist looking to further their career, in a patient facing role.  We are looking for enthusiastic and dynamic pharmacists, to join our expanding team of highly skilled experienced pharmacists and technicians.

This is a new 2 year fixed term post (that may continue beyond) for Medicines Optimisation in Care Homes (MOCH), and as part of the pharmacy care of the elderly team within the hospital.

Post holders will be responsible for delivering medicines management and optimisation services, working with local care homes, GP practices, community pharmacies and ward staff.  The role will include clinical medication reviews with care home residents, and professional advice regarding medication issues.  The successful candidate will work closely with local networks to optimise medications, ensure cost effective prescribing, improve clinical quality and reduce medicines waste.

You will have excellent interpersonal and communication skills, possess sound clinical knowledge, and thrive on a challenge!  You will be part of a progressive, agile, multi-disciplinary team working across care settings, and it is essential you can work both independently and as part of the team.  A qualification as a non-medical prescriber would be an advantage, but it is not essential, but must be registered with the GPhC.
