About the Role
The Associate Clinical Nurse Consultant (ACNC) provides clinical expertise and consultancy in collaboration with the Clinical Nurse Consultant (NG7). The role provides direct and indirect care as required and works with the wider health care team of the Heart Recovery Service (HRS) and Cardiology to ensure patient and unit goals are achieved. In supporting the CNC, the ACNC provides knowledge and skills, leadership, mentorship and direction to nursing staff and is responsible for the facilitation and provision of safe, quality and cost-effective nursing care consistent with contemporary standards and best practice in post discharge cardiovascular management programs.
In consultation and collaboration with the Clinical Nurse Consultant (CNC) and Nurse Practitioner (NP) within the HRS, this position will be responsible for the day to day management of the HRS secondary prevention and management gym programs. It will provide case management for all patients with cardiovascular disease including heart failure, acute coronary syndromes and post cardiac procedure or surgery attending the programs. This will include clinical supervision, assessment and management as well as patient and carer education for all patients undertaking this post discharge service.
